If the patient’s hospital account is covered by a company, insurance or HMO, the letter of Authorization (LOA) should be faxed to +63 (32) 253-5639, but the original LOA has to be presented before the discharge date. Please call our HMO/Insurance coordinator (local 5537) should you have any concerns regarding your charge account.
- If the patient is a PhilHealth, all requirements shall have to be completed/secured before the patients discharge date. You can ask PhilHealth, all requirements shall have to be completed/secured before the patients discharge date. You can ask PhilHealth Representative (counters 8 & 9) for any inquires.
- An amount shall be charged to your bill upon borrowing of items from the nurses’ station (e.g. TV remote control, drawer keys). The amount shall be deducted once the borrowed item(s) have been returned.

DISCHARGE PROCEDURE:
Step 1 Go to the Nurses’ Station, verify that your doctor has given the “May go home” order and obtain the following, as applicable:
- Discharge Instructions
- Medical Records (Certified True Copy)
- Philhealth Forms
Step 2 Return any borrowed item (remote control and keys) at the Nurses’ Station before proceeding to the business office.
Step 3 For PhilHealth members, proceed to Philhealth section (Counter 8 & 9) together with fully accomplished forms.
- PHIC Form 1 accomplished by the employer
- PHIC Forms signed by the attending physicians.
- PHIC Form 3
- PHIC Form 4 (for maternity cases only)
For incomplete PhilHealth requirements, you have to settle your account at the Billing Section in full. You can file your PhilHealth requirements after the discharge for direct refund from PhilHealth at a later date. |

|
Step 4 Proceed to Cashier Billing Section (Counter 3 to 7, Ground Floor, Bldg. C to any open satellite counter) for computation of final hospital bill.
- Senior Citizens discount (if applicable may be granted upon full payment and presentation of an identification card or document that establishes that the patient is a citizen of the Republic of the Philippines and is at least sixty (60) years old.
Step 5 A gate pass shall be given to you by the Billing Officer or Cashier upon your discharge.
Present this gatepass to the nurse-on-duty upon check-out and surrender it to the security guard upon leaving the hospital. top |

- Discharge cut-off time is at 12:00 noon. A fractional amount shall be charged to the account if discharged after the cut-off time.
- Please inform admitting section if you want to check out after the cut-off time.
- All major credit card payments are accepted CHECK PAYMENTS ARE NOT ACCEPTED.
- For an updated Statement of Account, you can request for it from the Billing Section from 3:00 p.m. to 5:00 p.m. or check the automated billing inquiry (Info Touch Machine- ITM).

- ATM machines are available at the Ground Floor Building C.
PHARMACY
- The hospital pharmacy located at the lobby of Building AB beside the entrance is open 24 hours seven days a week.
- Cash purchases at the hospital pharmacy shall enjoy a discounted rate for all medicines.
MEDICAL RECORDS
- Photocopies of medical records and medical certificates can be furnished by the Medical Records Section. Coordinate with the Nurses’ Station prior to discharge if you need a copy. A “Release of Information Consent Form” shall be signed by the Attending Physician and the patient prior to release of records.
- A courier service is available upon request. Additional charges incurred shall be included in the patient’s bill.
VISITING HOURS
- General
- 9:00 a.m. to 9:00 p.m.
- Children below 9 years old are STRICTLY NOT ALLOWED
- A Maximum of (2) watchers at a time are allowed for each patient. Two passes will be issued.

|
- Intensive Care Unit (ICU), Coronary Care Units (CCU) and Pediatric Intensive Care Unit (PICU)
- 9:00 a.m. to 9:00 p.m.
- ICU viewing area shall be closed from 9:00 p.m. to 6:00 a.m. to all visitors that does not have the watcher’s pass.

- Watcher’s Pass
- A pass is required to all watchers who will stay with the patient after 9:00 p.m. A watcher’s pass is given to the patient upon registration at the Admitting Section.
|
|
|
|
|
|
- Housing and Security
- Hospital staff shall collect all food trays inside the room.
- Cash and other valuables may be deposited to the cashier for safekeeping.
- All private room doors shall be kept locked at nightime.
- The hospital is not liable/responsible for anything lost during your stay.
|
|
|
|
|
|
- Other Charges
- Incoming calls are free of charge. All outgoing local calls shall be charged accordingly.
- Ambulance services are provided for a corresponding fee.
|
|
|
|
|
|
- Canteen
- Hospital Canteen is located at the Ground Floor of Main Building (near Medical Records section).
|
|
|
|
|
|
- Parking
- For your convenience, the Hospital offers a parking services at P10.00 per hour from 6:00 a.m. to 8:00 p.m. The fund collected will be donated to the Cebu Filipino-Chinese Volunteer Fire Brigade.
- Overnight and basement parking is strictly prohibited.

|
|
- Advance Directives
- To preserve the patient’s dignity and rights, Chong Hua Hospital has made available to patients who chose to sign an advance directive or living will, legal documents which will direct the attending physician and CHH staff, regarding preferred intensity of care including orders regarding life-sustaining treatment.

|
|
|
- Others
- Strictly NO SMOKING within the hospital premises.
- ALCHOLIC BEVERAGES ARE NOT ALLOWED.
- FIREARMS SHOULD BE DEPOSITED to the Security Officer.
